Fix hda_dsp_stream_init() error handling (leaks!)#273

The code in hda_dsp_stream_init() does not unwind memory allocations on errors.
Add as issue since the code is pretty complicated and needs attention/tests.

    closing for now, it's probably less complicated to free everything on error or remove.
