Polishes the connection dialog, host details, and file explorer menus. The saved-host selector now has a separate New host button and less unused space. Host details is smaller and presents tool availability as cards. File context menu actions have icons, including the new replacement and folder deletion actions from PR #32.
Rebased onto current main after PR #32 merged. Validation: TypeScript build and 414 Vitest tests passed; the production Vite build passed before the rebase.
