Create-issue flow is missing media wiring. On the new-issue page, MarkdownEditor is only given value / onChange / mentions — there is no media prop or onPaste handler, so paste and drag-and-drop never hook into uploads there. See apps/dashboard/src/components/issues/new/new-issue-page.tsx vs the issue/PR comment composer in DetailCommentBox, which uses useCommentMediaUpload and passes media + onPaste into MarkdownEditor. Thread replies reuse the same hook in comment-reply-form.tsx.
If paste/drag on comments also does nothing or errors, uploads depend on the comment-media API and R2 setup (POST /api/comment-media/upload). Local/dev notes are in .dev.vars.example (R2 / R2_PUBLIC_BASE_URL). The feature landed in #143.
So for implementation: extending the new-issue form with the same useCommentMediaUpload + MarkdownEditor props as DetailCommentBox should cover the “paste images in issues” (description) case; comment/PR paths already have the hook if the backend is configured.

Update: it works for comments. still not sure about PRs
