fix(server): Codex /compact no longer adopts a goal turn that starts first - #18912
A manual Codex /compact could bind to an autonomous goal turn that started first, including a goal continuation held by the adapter. That could settle the compact on the wrong turn or keep it running after its actual native turn finished.
Separate pending compaction from ordinary turn starts. When a goal can compete for ownership, classify the native turn from its first item and keep known turn ownership intact. Manual compaction completes independently of goal holds. Keep the start cancellable until its native turn is published, retain stopped-goal intent across later compacts, and fail an unclassified start when the Codex transport exits.
Rebuilt from @voltcrash's #18912, preserving contributor history and merging main at f740a97.
Verification:
The original added three adapter cases. The replacement has eight cases over main in the existing adapter suite, plus one shared-client transport-exit regression; no new test files or fixtures.
Before: the replacement ownership tests fail on main; the original PoC fails the held-goal case. Later Stop, itemless-terminal, and second-compact regressions reproduced before their fixes.
After: 177 adapter and 3 client cases pass; 24 focused orchestration/replay cases pass. The selected client/protocol/stdio suites pass 25 cases. Scoped server and client-package typechecks, targeted lint/format, and whitespace checks pass; two existing unused-variable warnings remain.
Six independent full-diff review rounds with Opus and Sol completed; the final round found no blockers. Fixes cover wrong-turn ownership, cancellation before binding, itemless terminals, transport loss, and stopped goal continuations arriving after another compact.
Proof is headless replay and client/transport execution. No live Codex account or app flow was exercised. macOS, Windows, Electron, iOS, Android, and remote connections were not tested directly.
Known limits of the pinned Codex protocol:
An unknown goal turn whose first item is automatic context compaction is indistinguishable from the requested manual compact and can consume its attempt. Automatic compaction on an already known turn keeps its owner.
With a competing active goal or retained goal Stop intent, an unknown itemless failed or interrupted turn can be either the compact or a stray goal turn. Classification can remain pending until the actual manual compaction is identified, Stop, or transport loss. This limit can persist across later compacts until an ordinary turn starts or late goal work is contained. Plain-thread itemless failure/interruption is covered. Stopping an unclassified compact can also pause an active goal because the next native start has no reliable owner marker.
Held-goal plus pending-compact reachability through normal orchestration remains unproven; the adapter contract requested by the original PR is covered. This does not claim a global solution to protocol ambiguity.
Landing and approval decisions remain with the coordinator. Unmerged.
Models/harness: tests by GPT-6 Astra, implementation by GPT-6.1 Sol, independent reviews by Claude Opus 5.5 and GPT-6 Sol; Codex and Claude harnesses in T3 Code.
Modernized against upstream 15fe28327278, preserving its turn-ownership guard and replay hooks. vp run check:changed upstream/main --concurrency 1 passed, including affected typechecks and the adapter/client regressions. The existing review concern about awaiting an interrupt response during cancellation cleanup remains open; this modernization does not claim to resolve it. No new live-provider verification was performed.
Macroscope's review found this PR approvable — This is a localized Codex orchestration bug fix that changes manual compaction turn ownership without altering schemas, defaults, deployment, or unrelated providers. The added replay tests cover the notification races and goal-continuation path, keeping the change within the straightforward, self-contained bug-fix category.

Walkthrough
CodexAdapterV2 now classifies native turns against pending compactions and goal state, including turns deferred around Stop or compaction cancellation. Tests cover those paths and verify that client termination errors reach remaining and later waiters.
Changes: Codex compaction ownership
Layer / File(s)
Summary
Native turn ownership and classification apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts
The adapter tracks pending compactions, stopped roots, unclaimed turns, and manual compaction turns. It classifies root-turn notifications and retains goal holds while a turn awaits ownership.
Compaction requests and goal Stop apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts
compactThread waits for compaction classification or client termination and handles cancellation. Goal Stop routes a deferred unclaimed turn to goal continuation when appropriate.
Replay coverage and termination waiters apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts, packages/effect-codex-app-server/src/client.test.ts
Replay tests cover manual compaction ownership, cancellation, itemless terminal outcomes, and goal Stop. Client tests check that remaining and later termination waiters receive the same input-stream error.
This change stops a manual Codex compact from adopting a competing goal turn. Two narrow edge cases remain. An itemless failed or interrupted compact that competes with a goal can stay pending until Stop or disconnect, which the PR documents as a Codex protocol limit. Cancelling a compact can briefly stall notification handling for up to 10 seconds. Neither affects the main compaction flow, so the risk of merging is low.
Security architecture risk: Low · up to be01a
The change is limited to Codex session handling and does not appear to grant new access or privileges. However, a compaction that ends before producing an identifying item can remain pending when a goal competes for ownership, leaving the run’s lifecycle unresolved.
Retained concerns
Medium · reliability · inferred: With a pending compaction and a competing active or held goal, turn/started defers ownership. If that native turn then fails or is interrupted without a contextCompaction item, terminal classification does not resolve the compaction waiter or remove its pending record. The compact operation can remain pending after native termination, and the stale record continues influencing later turn ownership. The added itemless-terminal tests exercise only the uncontested path, which claims ownership immediately. Cancellation and client exit provide recovery, but the native terminal event itself does not close this transition.
Security review details
Security Blast Radius
inferred — The identified lifecycle concern is bounded to native-thread ownership within a Codex session. Client construction creates a separate termination Deferred per instance, and the session factory scopes a spawned child process and client together. No cross-tenant, cross-service, credential, or infrastructure authority expansion was established by the inspected change.
Trust Boundaries and Controls
observed — The new waitExit capability observes termination only; it does not send RPCs, control a process, or alter credentials. Existing request encoding and response decoding remain separate. Actual protocol termination fails pending requests and closes outgoing transport before notifying the client termination observer.
Resilience and Maintainability Implications
inferred — The remaining concern is ownership and recovery drift, not a verified privilege bypass: positive identification can settle the compaction, while an ambiguous native terminal leaves the waiter pending. Explicit interruption and client termination remain recovery paths, so the evidence does not support a service-wide availability claim.

The real Codex app-server generated the competing goal start inside the compact window in 8/8 active-goal reconnect trials on each of 0.162.1 and 0.163.0, versus 0/4 paused-goal controls on each. All 24 direct native compactions completed.
The unmodified installed T3 Beta 0.1.0 server with Codex 0.163.0 reproduced the adapter failure: T3 bound the compact run to goal turn A, persisted the run as interrupted, logged the actual compaction turn B as codex-goal-turn-without-run, and sent turn/interrupt targeting B. A subsequent warm, paused-goal compact completed. A separate T3 attempt using 0.162.1 missed the narrow timing window and completed; this is not a claim that every reconnect fails.
These are isolated-state experiments with a loopback mock model endpoint. Codex generated the native turns; no native turn events or RPC errors were injected in those experiments. This verifies the reported failure on the installed baseline. I have not live-tested this PR's head, and the exact historical Alpha error wording remains unverified.
The existing PR covers this defect, so I am adding the evidence here instead of opening another PR.
Reproduction/investigation: GPT-6.1 Sol through the Codex harness in T3 Code.
Related post-compaction lifecycle evidence from an installed Nightly (0.1.3-nightly.20261023.3592):
A user explicitly submits /compact while a Codex goal is active.
The native contextCompaction item and its actual compact turn complete successfully.
An ordinary autonomous goal turn starts immediately after completion, followed by further ordinary goal turns.
T3 keeps all those successors under the original maintenance-origin run. Explicit MCP mode=steer is downgraded to queued, and queued-message promotion continues to refuse with "Wait for context compaction to finish before steering the thread."
The observed delay was subsequent goal work under a completed compact's run, not a native compact still taking hours. We joined durable item/run metadata to native protocol event types and timestamps; the thread is now stopped. No private prompts or identifiers are included here.
At be01ac2, this PR's manualCompactionTurns exclusion from holdsForGoal addresses the source-level mechanism. The new ownership replay I inspected exercises a competing goal before the compact. Please also pin the opposite ordering: actual manual compact first, successful terminal, then a distinct ordinary goal successor while the goal remains active.
Acceptance should establish that the completed maintenance run cannot indefinitely remain the active steering target; any continuing goal work has a valid ordinary run owner, or is explicitly contained with a visible paused state. Before compact completion, retain maintenance queue/refusal. After a valid ordinary successor is current, exercise MCP-equivalent steer and promotion of a retained queued message, checking one delivery to the correct successor, no silent loss/duplicate and no steer into the compact turn. Preserve Stop, /logout and the already-covered ownership races.
I have not executed this PR head or changed the installed app. #24138 is the complementary automatic-compaction client fix and does not change this server path. Reusing this existing lifecycle PR rather than opening another implementation.

Update: required-check readback at be01ac2 has five executed SUCCESS statuses and three SKIPPED statuses, not eight executed suites. GitHub currently reports CONFLICTING/DIRTY against main52628d863e977e1a8b16644141fd00ba96640b16 (merge basef740a97947cc701859040e530e7fe07b96d02ae5, 904 commits behind). A bounded git merge-file --diff3 comparison of the four changed public files finds five textual conflict hunks in CodexAdapterV2.test.ts; the other three files merge textually. This is not a full checkout merge, test run or evidence that the behavior is safe on current main. Please preserve the ownership/Stop/transport regressions during maintainer reconciliation and rerun the affected suites on the new exact head.

Fork stopUnclaimedTurn during cancellation cleanup. Do not await it while holding turnTerminalizationPermit.
compactThread performs cancellation cleanup while holding turnTerminalizationPermit. stopUnclaimedTurn waits for the turn/interrupt response. The protocol reader processes notifications before it reads later responses. If a permit-taking notification arrives first, the reader can block on the permit and the interrupt request can time out after 10 seconds.
