Auto-name sessions and allow renaming

Sessions should get meaningful names automatically after the first message (summarize via agent or extract keywords, fallback to truncated message). Users should be able to rename: double-click in sidebar for inline edit, or right-click > Rename. Name persists in DB and shows in both sidebar and session header.
