### Describe the issue

Duplicate Form Code and Form Name Should Not Be Allowed

### Steps to reproduce

1. Go to the Observation forms
2. Create form
3. Click Begin new form
4. Select from "Form type"
5. Fill "Form code"

### Expected behavior

Currently, the system allows multiple forms to be created with the same Form Code and Form Name. This can lead to confusion, data integrity issues, and difficulties during form selection or filtering

OS: EMUI 12.0.0
Device: Huawei Mate 10 Pro

This issue affects filtering and searching.
