🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win
Stop scanning mode options at --.
Commander treats arguments after -- as operands, but applyModeFromArgv continues to interpret them as options. (raw.githubusercontent.com) For example, --mode agent --input-json { -- --mode=human selects human mode before JSON expansion fails. The invalid_json error then loses the requested agent-mode JSON format. Stop the scan at the -- sentinel.

🎯 Functional Correctness | 🟠 Major | ⚡ Quick win
Treat successful commander.help exits as help, not usage errors.
Commander emits commander.help for the help command, including clerk help, and can assign that outcome exit code 0. (raw.githubusercontent.com) This predicate misses that code. runProgram therefore prints help and then reports a usage_error with a nonzero exit code. Handle commander.help according to its exit code so successful help exits cleanly; retain error handling for help displayed because a command is missing.

Add a CLERK_MODE=agent parser-error integration case.
The changed test covers only --mode agent. The integration harness reaches runProgram, but its mocked mode.ts ignores process.env.CLERK_MODE. A regression in environment-selected agent mode can therefore pass. Add the environment case and make the mock preserve --mode precedence.

Summary
Fixes #559.
--mode agentwas applied in the commandpreActionhook, so--input-jsonfailures that happen before parse still used the TTY default. Commander also printed unknown-command / unknown-option errors itself and exited, so they never went through the JSON error handler.--modeis now read from argv before--input-jsonexpansion. CommanderexitOverridesends parse errors throughreportErrorwithusage_error. Help and version still exit cleanly.Test plan
