Test the moved org's v2 MCP URL

Tests for the PR below.

• e2e (cloud/v2-moved-notice.test.ts): the move answer's MCP URL is exactly v2's <origin>/mcp (not an org-pinned path). The notice shows that URL and "Connect your agents to v2 instead, and choose when you sign in:". The Connect card and /setup-mcp show the v2 command, "Choose when you sign in.", and the old v1 URL labelled. The long-name check includes the URL row. An MCP client's initialize result has no instructions before the move and the exact move instructions (URL and org to choose) after it, and the tool list stays the same.
• e2e: an MCP client connected before the move gets HTTP 404 on its next request, both while its session is still in memory (after the re-read window, which e2e sets to 1s via MCP_V2_MOVED_RECHECK_MS) and after the session's runtime was released and restored. Its new session's initialize carries the notice, and that session keeps working past the re-read window.
• e2e: at 320px wide, a 45-character unbroken v2 slug stays inside the notice and the Connect card.
• Unit: the session DO ends a session whose client must re-initialize (also after a restore), keeps it while an execution is paused or a request is running, never ends it for a caller that doesn't own it, and reports a live initialize to the host. The decision rule: re-initialize only when the org has moved and the client got a different notice or none.
• Unit: the v2McpUrl parsing, V2_MCP_ORIGIN validation, the shipped config, and the instructions text and read (unset var, failure, timeout).
