Redirect Mentor Request link to internal app route #1031

Why is this feature being added?
When the new form at /mentor-request was initially rolled out, there were issues with the Back-end communicating with the Airtable API, so the link on the profile page was temporarily changed to the URL shortener at http://op.co.de/mentor-request (in #1026). We've since fixed the issue, so we can point directly to the correct route within our app.
What should your feature do?
Change the 'Request a Mentor' icon URL so it links directly to the /mentor-request route within the Front-end app.
