Subject: [PATCH] ci: add minimal GitHub Actions test matrix

Additive only: ubuntu + windows, Node 20/22, Python 3.10-3.12.
Runs tsc, mocha suite, and high-severity audit gate.
AppVeyor untouched; no existing file modified.
